Kindred Hearts is proud to offer Enhanced Care Management (ECM). ECM helps people who could use extra support with health and daily life.
Whether you’re navigating multiple diagnoses, high-utilization of emergency services, or facing barriers to basic needs like housing and food, ECM steps in with compassionate, coordinated support that’s tailored to your goals and your story.
You may be eligible if you:
- Are an adoptive family, kinship caregiver, or legal guardian
- Are experiencing homelessness or housing instability
- Have ongoing mental health or substance use needs
- Have complex medical or health needs that require extra support
- Are transitioning from incarceration or a group facility
- Need help staying healthy, safe, and supported at home
What ECM Offers:
- A dedicated Case Manager who builds a personal relationship with you
- Help accessing and understanding your benefits
- Coordination with doctors, specialists, schools, and social workers
- Advocacy and emotional support to reduce burnout and stress
- Long-term planning for success in the home, school, and community
ECM is open to Medi-Cal beneficiaries who qualify — and yes, you can still access ECM even if you’re not part of an adoptive or foster family. It’s about making sure that no one falls through the cracks.
