Wraparound is a family-centered planning process designed for adoptive families navigating complex challenges. It offers structured, personalized support that brings together a team of professionals, peers, and family members to create a plan that promotes healing, connection, and long-term success.

This isn’t a one-size-fits-all approach—it’s a commitment to walking alongside families as they address emotional, behavioral, and systemic challenges that can emerge after adoption.

4 Phases of Wraparound

Engagement

We begin by building a foundation of trust with your family. During this phase, we listen to your story, clarify expectations, and explore your family’s strengths and needs. Our goal is to establish safety and connection—because the process only works when you’re truly heard.

Initial Plan Development

Your family’s team comes together to design a customized Wraparound plan. Built on your vision and values, the plan includes practical strategies, supports, and measurable goals that reflect your unique circumstances and priorities.

Plan Implementation

We put your plan into action. Through regular meetings and ongoing collaboration, we celebrate progress, tackle obstacles, and adapt the plan as needed. This phase is all about movement—toward healing, stability, and success.

Plan Transition

As your family grows stronger, we prepare for the final phase: transitioning out of Wraparound. We ensure long-term supports are in place and help you confidently move forward with lasting tools, resources, and a strong sense of community.

FAQ’s

The primary qualifying criteria are that the youth has been adopted out of foster care and is experiencing mental health or behavioral challenges. Each county of origin makes the final decision regarding your family’s Wraparound qualifications.

There is no cost to your family to receive Wraparound. Funding comes from state Adoptions Assistance, and through billing insurance. Your family continues to receive your monthly Adoption Assistance Payment and it does not interfere with existing family services.

 

Contact Kindred Hearts to request an intake session to see if our services are a good fit for your family. Final approval for funding will come from a County Adoptions Worker, though we work closely with your family and your worker on the approval process.
